• Follow-up pregnancy tests, office visits, ultrasound examinations, or laboratory testing to monitor a resulting pregnancy, or procedures related to pregnancy.

• Unless already paid when initiating an A.R.T. cycle at F.I.R.S.T., a one-time, NON-REFUNDABLE Administrative Surcharge:

Unless we are contracted with your insurance carrier, fees must be paid in full prior to beginning medication for treatment, at the time of your start-up appointment. You may be eligible for payment plans available through outside agencies. Unless you have paid in full for our money-back guarantee plan, there are no refunds if the procedure does not result in pregnancy or a live birth. If a repeat procedure is necessary, you MAY be eligible for a discount for a repeat cycle. F.I.R.S.T. accepts cash, local checks and credit cards. If any of the services in the fee schedule above are not rendered because of cancellation of your cycle, you will receive a refund for the difference. As a courtesy, F.I.R.S.T. may help you bill and collect from your insurance company by preparing your claim form; however, we cannot assure you of insurance reimbursement.
